The New Customer Application and Autopay forms can be completed by property owners and sent to us electronically following a few quick steps. Tenants do not need to complete any BCSS forms. If you are a tenant who wishes to have your name added to the sewer service account, please contact our office.
If you do not want to complete them electronically, you can print a form by clicking on “Other Actions” then “Print & Sign” after opening the form. After printing it, you can scan and email it to firstname.lastname@example.org, fax it to (251) 971-6039, or mail it to P.O. Box 1628 Foley, AL 36536.
